Sevita is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our Healthcare & Medical Services team, providing home and community-based care to individuals of all ages and abilities. As an LPN with Sevita, you will play a key role in delivering person-centered nursing care that supports individuals living with behavioral health needs, brain injuries, developmental and intellectual disabilities, and complex medical conditions. In this role, you will provide direct, hands-on nursing care in homes and community settings, including monitoring vital signs, administering medications and treatments as ordered, coordinating with physicians and registered nurses, and documenting care accurately in electronic health records. You will assist with health assessments, support individuals with activities of daily living, and respond promptly and appropriately to changes in condition, always maintaining a focus on safety, dignity, and quality of life. You will collaborate closely with an interdisciplinary team—including RNs, therapists, behavioral health professionals, caregivers, and family members—to implement individualized care plans that reflect each person’s unique goals and preferences. Your ability to communicate clearly, demonstrate empathy, and build trusting relationships will be essential to helping the people we serve live as independently and fully as possible in their communities.
